Therefore, we offer a clear list of all the prices we charge for services. Letting patients know the true cost of healthcare is unheard of in our current medical economy. Our price list is based on covering the cost of offering these services to our patients, therefore, the prices are subject to change as market forces change. Please talk with your DPC physician about the current price of any specific test.

Memberships
- Newborns through 2 years of age are $75 per month
- Children 3-18 years of age are $60 per month
- Young adults 19-39 years of age are $79 per month
- Adults 40-66 years of age are $89 per month
- Seniors 67 years of age and older are $99 per month.

- Metformin ER 500mg #100 for $5.60
- Montelukast 10mg #90 for $7.29
- Sildenafil 20mg #90 for $12.60

Labs
- Labs included in membership (see Basic Membership for details)
- Send out testing: CBC, Lipids, Complete Metabolic Panel, TSH, Insulin, HgbA1c, PSA, Cardiac CRP
- In house testing: Strep test, urine pregnancy test, urinalysis
Procedures
- Procedures included in basic membership
- EKG, PFTs, Cerumen removal, Suture removal, hearing screen, vision, screen
- Additional procedures:
- Osteopathic Manipulative Medicine with Dr. Howell: $60 per session
- Breathing treatments: $12
- Punch or shave biopsy: $50; pathologist fees not included
- Laceration of suture: up to $200 depending on complexity
- In office allergy shot administration: $20/month regardless of visits
- Holter Monitor: $75
- Vitamin B12 injection, 1000mcg: $15
- Adacel (Tdap, tetanus, diphtheria, acellular pertusis): $60
